If you are an active participant at Good Shepherd, you may request a wedding at Good Shepherd Church by following these steps:

Complete this form to officially request Good Shepherd for your wedding.
You will receive notification of whether or not your wedding date is confirmed within 2 weeks of your submission of this wedding request form.
Once the wedding date is confirmed, a $200 deposit is required to secure your wedding date. The deposit is refundable up to 30 days prior to your wedding date.
